Food Establishment Information

Business Certificates

All New Food Establishments must file an application for a Business Certificate with the Town Clerk's Office.

  • Prior to the issuance of a Business Certificate, the applicant will need approval from the Inspections Department and the Health Department.

Food Establishment Plan Reviews

All new Food Establishments must file an application for a Food Establishment Plan Review.

  • The application for a Food Establishment Plan Review must be filed through our online portal.  Please CLICK HERE for the online portal.
  • A hard copy of the application may be found HERE.  This hard copy is to help the applicant prepare for the online application.   PLEASE NOTE:  The application MUST be filled out on our online portal.
  • Once your Food Establishment Plan Review is approved, there will two (2) inspections performed at your location.  The first inspection will be after all the physical equipment is in place and the second inspection will be after your first food delivery.
  • After the first inspection is completed and approved, the applicant must apply for a Food Establishment License prior to opening the establishment.
  • The Food Establishment Plan Review is generally a one-time application.  However, a new Food Establishment Plan Review may be required if you plan to remodel your establishment, if any repairs need to be completed or if there is any change in use.
