COME GET YOUR FLU VACCINE! Flu Clinic Schedule - Middleton Residents Only
The Middleton Health Department will sponsor flu clinics on the date, time and location listed below. ** Please note that because of limited pediatric vaccines, we request that those under the age of 5 years old receive their vaccine from their pediatrician. All children under 18 years must be accompanied by a parent at the clinic. State supply of vaccines has been greatly reduced so clinics will be held until the vaccine is depleted. Please bring insurance information with you to the clinic. We are using a newer software called COLOR, you will need to pre-register prior to any immunization clinics. The link is below.

Homebound individuals, who require a home visit, should contact the Middleton Council on Aging (978) 777-4067 to schedule an appointment. If you need assistance with filling out the form via COLOR, please contact the Tri-Town Public Health Nurse at (978) 766-6284
- Walk-ins are accepted, however, please be prepared to wait, as it takes a few minutes to register each person. Walkins may be limited based on vaccine availability.
- Please ensure when you sign up in Color your name matches the name on your insurance card. (IE if the name on your insurance card reads "Robert A. Smith", don't sign up using the name "Bob Smith")
- Please bring your health insurance card to your appointment.
- Don’t forget to wear a short sleeve or loose-fitting shirt so your upper arm is free.
- Regular, and High dose flu vaccines for ages 65+ are available.
- Flu mist for children is also available.
